I don't change my needle often enough and I often sew with no bobbin thread. I think the most common mistake I make though is trying to sew past my limit.....often too late at night. It often comes back to haunt me with the seam ripper then next day!

I think the most common mistake would be in getting an accurate 1/4 inch seam allowance.

Oh that is easy, incorrect fabric placement. Whether is it not right sides together or a wrong row sewn upside down, or 2 blocks sewn together wrong. I have named my seam ripper. Oh yeah, he is the most used device I have. Yes, it is a he because I make him work!!! His name is Jack....Jack the seam Ripper.

I have to agree, it's keeping an accurate 1/4 inch seam.

My most common mistake is not sewing right side to right side....that's why my seam ripper is my best friend!

Sewing when you are tired. Definitely not a good idea.

I guess I would say - in quilt sewing - it would be not having a consistent 1/4 inch seam allowance. Honestly though, I constantly make a new kind of mistake all the time - good golly, Miss Molly, where is your head!!

I think the biggest mistake is not taking the time to read through your pattern to be sure you don't just assume it is being made the same as another you've done in the past. Just like reading a recipe while cooking, sometimes even with the very same dish, the directions are different so it's best not to just assume you already know and read through at least once. That helps lessen the chances of having to spend time ripping out seams.
