I generally just make a label for the back with the following info:
Name of the quilt (if it has one)
Who Pieced and Quilted
Date Completed
Special message to recipient
Name of Designer and Pattern, if known
The Quilting for the Rest of Us Podcast did a great episode on Labels and why it is important to document our quilts! I recommend giving it a listen if you ever have time. I never realized how important it was until I heard that episode and decided to get better at it.

I also make a label with the same information as Katie (above). In a pinch I have been known to sign the back with a Pigma pen, which holds up well through many washings. When I hand quilted everything I would attach the label to my quilt back before quilting so the label could not easily be torn off if my quilt was ever stolen. My DH convinced me I was was getting a little psycho about my quilts so I now attach the lable after the quilt is complete. Barb

I downloaded some embroidery designs from http://home.comcast.net/~quilts4u43/label_templates.htm (thanks to Sandy) and converted them to JEF format, (I have a Janome embroidery machine, added some text and sewed them onto the back of my quilts.
I had a bit of a stockpile happening with no labels, so spent a good weekend making them, fun... and makes the quilt complete.

One thing I've been doing is...making an extra block that matches the front. Depending on the colors, I might piece or applique a white or snow solid in the middle of the block. Then I applique it somewhere in the middle of my backing, before I quilt it. I just do it as part of preparing the backing for the quilt.
