I think for guys you have to be more aware of the colors and patterns of the fabric (not too many guys want flowers) than you do about quilt patterns. Stars always seem popular with the guys though.

Double slice layer cake in Pine Island batiks. I made one for DH for Christmas. We both love it. I used a bear and pine tree panto.
I just made one out of Jenny's tutorial for the Coin quilt with little pizzazz. It goes together quickly and I think the geometric pattern is nice for guys.

The double slice layer cake is another fun one and could look manly with the right fabric. And, of course, stars are always a good choice.
I am not sure about pattern but be sure to choose colors that can get dirty. My guys are always eating and spilling. I did a tumbler quilt for my oldest son and a fence rail for the 2nd. Still trying to decide on a pattern for son #3
All good suggestions above. My son likes the zipper quilt. Perhaps choose a more modern line that would look good with a dark solid for background color.

However, I do like the other suggestions. I was thinking about this today actually as I need to start one for my soon to be son-in-law. Choosing color is problematic because he only said black when I asked what color he would like. When I asked what else, he said grey. LOL
So in any case, I've been thinking about doing a tumbler, or perhaps something in a log cabin but without being really square. I was thinking about using a tumbler as the center of a log cabin and possibly in the courthouse steps version in shades of grey and dark blue with the center tumbler in black.
I've found this one recently. It would be easy enough to recreate with different black and grey fabrics. Maybe add some interesting quilting. "Little Black Dress 2" might be the fabric line to choose.
I would definitely ask your grandson what colours he likes. Also show him different patterns that you would be able and willing to do and let him choose (or let him google it himself). You might be surprised by his choice!
