I have done it both ways. I am less likely to use batting if the quilt is made of flannel.

I piece a cotton top with a flannel back and no batting, just the way I do it...
Quilts may come, and quilts may go, but the stitches stay the same.
don't forget most babies in the nicu are in incubators so the quilt shouldn't be to warm
Most of the patterns and sites that I have seen for preemie blankets are without batting. The reasons given were 1) they are too warm and more importantly, 2) they are too heavy on such a little baby.

I followed the pattern requested from NICU nurses.....no batting....too heavy for little ones. It also specifies very little quilting stitching...so no rubbing on delicate skin.
